Lead Professional

When a child or young person may require input from more than one service the aim is for a practitioner to act as a lead professional to:

• Provide a single point of contact trusted by children and young people and families able to support them in making choices and in navigating their way through the system.
• Ensure that children and young people and their families get appropriate interventions when needed, which are well planned, regularly reviewed and effectively delivered
• Reduce overlap and inconsistency from other practitioners

The aim is that the majority of practitioners in Slough working with children and young people will be expected to act as a lead professional for a child or young person and act as the coordinator of services and the single point of contact.
